Incident Reporting Notes

For app-related incidents. Not a substitute for emergency services.

When to report

  • Safety: the app produced harmful, unsafe, or policy-violating output.
  • Privacy: you suspect your account data was accessed, leaked, or mishandled.
  • Security: you discovered a vulnerability, bug, or suspicious behavior.
  • Abuse: another user used the app to harass, threaten, or harm you.

How to report

Email theconsigliereoffice@gmail.com with subject prefix:

  • [SAFETY] — harmful or policy-violating output
  • [PRIVACY] — data access or deletion request
  • [SECURITY] — vulnerability disclosure
  • [ABUSE] — abuse by another user

What to include

  • Date and approximate time (with timezone).
  • Account email used (if applicable).
  • Steps to reproduce, or the prompt and the response received.
  • Screenshots or copies of relevant messages.
  • Impact: what happened, and what outcome you are seeking.

What to expect

  • Acknowledgement within a reasonable timeframe (typically a few business days).
  • Triage and investigation appropriate to the severity of the report.
  • For confirmed security issues, please give us reasonable time to remediate before public disclosure.
  • For privacy/deletion requests, action within a reasonable timeframe, subject to identity verification.

Documenting workplace incidents

For incidents inside your own workplace (as opposed to incidents with the app), use the in-app Evidence Journal. Keep entries factual: date, people present, what was said or done, and any witnesses. Avoid speculation and labels. This makes the record usable if you later involve HR, a lawyer, or a labor authority.

Not for emergencies

If you are in immediate physical danger or a mental-health crisis, contact local emergency services or a qualified professional in your country. This inbox is monitored periodically, not 24/7.
